All plans require a one-time enrollment fee of $30.
The primary member must be between 18 and 65 years old and reside in the United States.
If the majority of members — or any member over 65 years old — or the primary member resides in regular-rate states, the family membership cost will be $85
Members 65 years old or younger may be included regardless of their relationship to the primary member.
Parents or the partner of the primary member may join up to 80 years old at the time of enrollment, with continuous coverage while the plan remains active.
Up to two additional people can be added under these special conditions.
